Questions

Do all thirty questions apply to every AI system?

No. Instructions for use, data governance, human oversight, logs, monitoring, incident reporting, accuracy, technical documentation, the quality management system and conformity assessment are high-risk questions. For any other system the answer is Not applicable, with the system's classification as the reason. Inventory, roles, literacy, prohibited practices and the Article 50 disclosures reach further.

Where do these questions come from?

We wrote them as the kinds of AI questions that enterprise vendor questionnaires and audits ask, grouped by topic. Each is tied to the provision of Regulation (EU) 2024/1689, as amended by Regulation (EU) 2026/1744, that it is about. They are not copied from any one questionnaire, and a customer's own wording will differ.

Is answering all thirty the same as complying with the AI Act?

No. An answer records what you do and the provision it rests on; it is not a conformity assessment, a certificate or an authority's view. A high-risk system shows conformity through the Article 43 procedure and the Article 47 declaration, from 2 December 2027 or 2 August 2028 (Article 113, third paragraph, point (c), as replaced).

How does Complipath help answer them?

Complipath keeps a register of your AI systems, each classified by fixed rules with the article behind it and confirmed by you. On Starter and up it answers a customer's questionnaire in the customer's own Excel, Word or PDF file, each answer with its source, and marks what it cannot answer "needs your answer".
