In Complipath (complipath.io), every assessment of an AI system is kept: running it again adds a new record beside the old one, with its own date, answers, engine version and who confirmed it. A contest, a lawyer's note and a provider's Article 6(3) claim stand on that record, and each assessment downloads as a file.
Every assessment a system has had, what differed between the last two, and which legal text each rested on.

Not in place. The database refuses any change to a recorded outcome; only the confirm and contest stamps may be added. To reach a different outcome you run the assessment again, and the new record stands beside the old one, so anyone can see what changed, when and on which engine version.
Contested, with the date and your reason, is stored on that assessment and shown under it. Nothing else moves: the class, its requirements and their dates stay as they were. Contesting is a state on the record, not a ticket to us; to change the outcome, run the assessment again.
No. A provider records its own conclusion that an Annex III system is not high-risk, with the condition it relies on and both provisos affirmed. The record says the conclusion is the provider's and has not been verified. The system stays high-risk in the register, and the Article 6(4) and Article 49(2) duties are added.
